Registration Information
Below are the registration forms required for students in Union School District. You may print out the forms, complete them and bring them to your home school along with proof of residency, the child's birth certificate and immunization record. If your child has been receiving special education services at their prior school, please bring a copy of their IEP as well.
Applications for the 2008-2009 school year will not be accepted prior to January 14, 2008.
